1. Black defeated Venom in a tournament final to become first PWF Northeast Champion.

2. It appeared that Venom defeated Black for the Northeast Title, but reviews of the video of the match revealed that Black's foot was on the rope as he was pinned. After the match, PWF Northeast Owner Mark Reality declared the Title Vacant.

3. Mark Reality declared the title vacant when the reigning champion Johnny Curtis no-showed a mandatory defense.

4. Black pinned Perez in a Fatal Four Way to decide the vacant PWF Northeast Heavyweight Championship.

5. This match took place at PWF New Jersey "Conception" in Clifton, NJ.

6. Triplelicious regains the PWF Northeast Heavyweight Title from Jason Blade at "Gold Rush". The title is  unified with the Mayhem Title to become the PWF Mayhem Heavyweight Title.

 *1. "Live Wire" Dave Padula defeated 20 other Mayhem Independent Wrestling stars in the first Toxic Waltz to become the first Mayhem Heavyweight Champion.

2. In a public forum, Padula quit Mayhem in a dispute with president Vinny D'Martino. Ivan Zangvine took the belt and named himself champion.

*3. Zangvine's claim was substantiated in the second Mayhem Toxic Waltz. Zangvine went through 15 Mayhem stars including future WWE Tag Champ Charlie Haas and future RoH Tag Champ Danny Maff.

*4. Angel was stripped by the Mayhem resurrection due to his inability to defend the title (and conflicts  with prior bookings). The Resurrection committee named Padula as the new champion, seeing as he was also the last Mayhem Hard-core Champion as well.

*5. Triplelicious regains the Mayhem Heavyweight Title from Ebony Blade at "Gold Rush". The title is soon thereafter unified with the PWF Northeast Title to become the PWF Mayhem Heavyweight Title.

1. Murphy scored the final pin-fall of a "Double Jeopardy" match over Phoenix.

2. Hudson defeated Richter after Knoxville was stripped due to injury.

3. Richter pinned Jason Blade in a Fatal Four Way with Mike Paiva and Paul Hudson. Hudson's Title was on the line.

4. Richter lost the title by pinfall, but revealed he had a rematch clause stating he could name the place and time for the rematch. He chose to take the rematch right after his title loss and was able to steal the title back with a ropes-assisted pin on Blade.

5. Kid Mikaze won the ladder match at "Genesis IV" unifying the PWF Northeast and Mayhem Junior-Heavyweight Titles, now recognized as the PWF Mayhem Junior-Heavyweight Title.

6. Pat Masters pinned Riot in a "fatal four-way" match to win Mikaze's title. Kid Mikaze and Nocturne were the other two competitors.

1. The Hard-Core Institute's Armadillo & Krazy were awarded the titles when The Showtime Posse was unable to contest for the titles.

2. At PWF Universal's "Big Impact" event in Pottstown, PA.

3. At NWA Wildside TV Taping in Cornelia, GA.

4. The Talent Exchange defeated both teams in a TLC Match for the titles.

5. Dean Ripley was forced to team in place of Johnny Curtis, who he had given the "night off". PWF-NE Owner Mark Reality forced him to defend the titles with Phoenix.

6. Owner Kyle Storm was 1/2 of the Tag Champions when buying the company. In the interest of fairness, he vacated the titles.

7. Chris Camaro was the final man left in the "Toxic Waltz" winning the titles for Thrillogy.

8. Jason Blade and the Sandman were asked to vacate the titles due to Blade's name not appearing on the original match contracts (He subbed for New Jack, who no-showed the title match with Intolerance).

9. Pride defeated The Logans in the finals of a three month long tournament to determine the new tag team champions.

10. Elements of Suicide defeat Pride and unify the PWF Northeast & Mayhem Tag Team Titles.

11. 1/29/04 it is announced that unified titles will just be recognized as the PWF Mayhem Tag Team Titles.

1. Demonica defeated Ariel and Val Dictorian at Genesis III to decide who would be the inaugural PWF Northeast Women's Champ.

2. Demonica was scheduled to defend the title in a three way against Bayless and Natalia. She could not defend the title due to an injury and the belt was put on the line between the two other opponents.

3. 1/29/04 It was announced that in line with the purchase of the company, the title would be renamed the PWF Mayhem Women's Title.

4. Natalia was never pinned by Demonica. This title-loss came as a stipulation for the eight-man mixed tag team match at "Driven". Evan Siks eliminated Natalia from the match. Demonica scored the final pin fall on Dan Freitas (along with some timely interference by Kris Frost) to win the match for her team. As stipulated, she was crowned the new PWF Mayhem Women's Champion.
